Questionnaire for Community Service Project

Assignment: Write a 3 to 5 page narrative describing your project, including its goals and outcome. The narrative must be typed, double-spaced, in font size 12, font type Times New Roman, and with margins 1 inch long. Answer the following questions in your discussion of you community service project:

What is the name of your site, the address, the phone number, and who is supervisor or contact person for the placement? How many hours did you spend at this site?

Why did you choose this site?

How does this placement align with your major?

What goals/objectives did you have when you started?

What skills did you bring to the project?

What did you accomplish? How did your project profit from your participation?

What advice would you give to other students considering volunteering at this site?

How did what you learned in class contribute to your community service experience?

Note: Be specific in your comments. Cite anecdotes or specific incidents during your time at the project that demonstrate your answer in a concrete manner.
